Enrollment Procedure

There is a registration paperwork packet that has to be filled out that includes:

-Davidson County Registration Form

-Bus Form

-Health Form

-Information sheet

-Other school letters

Also Need:

-2 proofs of residence (power, water, landline phone, cable, etc)

-Copy of photo ID-parent

-Certified birth certificate

-Legal Custody Papers(if applicable)

-Immunization record

If coming from out of state or enrolling in kindergarten, the student must have had a physical within the last year.
